This application is not a legal, binding contract, whereby filling it out does not guarantee pet adoption. By choosing "I Agree" you are agreeing that Warrior Dog Rescue may contact your veterinarian and references to verify your history regarding pet care and consistency. Please understand that we use this form as a part of our screening process, thus it may take from several days to several weeks before finalization of the adoption can occur. Any false statements provided on this application, during the interview process, or at any time during our decision making period, will result in an automatic denial.
